Membership Plan

• Adults: $149/month

• Two adults in the same household: $199/month

• Additional children (ages 0–18): $20/month each

Includes:

• Unlimited access to a Prepared RN via text, email, or phone (Mon, Tue, Thu, Fri)

• Care coordination: finding providers, scheduling follow-ups, organizing referrals

• Patient advocacy: explaining diagnoses, medications, and treatment plans

• Monthly nurse check-in call with personalized care planning

Member Add-Ons:

• After-hours support (evenings & weekends): $75/hr

• Written notes, care plans, discharge & care summaries: $50 each

Non-Member / One-Time Services

• Emergency Care Navigation: $149 flat fee (post-ER/urgent care consultation, virtual session)

• Care Coordination & Advocacy:

  • Business hours: $75/hr

  • Evenings & weekends: $100/hr

• Written Notes & Care Plans: $75 each
